  3. Sign the Attendance Policy and other documents

  4. Attach any reports, IEPs or other evaluations you’d like your therapist to review

  5. Request a referral from your pediatrician for “OT evaluate and treat” with your child’s diagnosis be faxed to us at 206-735-3778

  6. Here’s a short video to share with your child if they’re feeling nervous about coming to a new place.
